    Vertigo & Balance Program
     
    The Vertigo and Balance Program at HEALTHSOUTH Outpatient in Ludlow, Massachusetts evaluates patients with persistent disequilibrium using a comprehensive assessment of balance, gaze stabilization, strength, endurance, range of motion and overall function in daily activities. This comprehensive evaluation allows for a timely, cost-effective, accurate assessment of the condition, thus enabling the appropriate intervention.
     
    Indications for Referral
    Individuals who will benefit from an evaluation include those with:
    • A history of falls due to vertigo or dizziness
    • Vertigo or dizziness not responsive to usual forms of medical management
    • Significant balance dysfunction in conjunction with normal ENG
    • Gait or balance disorders insufficiently explained by neurological examination
    • Postoperative inner ear surgery
    • Severe, disabling disequilibrium
    • Identification of symptom magnification

     
    Assessment Battery
    A complete battery of basic balance function testing may be necessary to fully assess the cause of dizziness or disequilibrium.
    Tests may include:
    • A thorough medical history
    • Vestibular ocular reflex testing
    • Specialized physical therapy evaluation

     
    Rationale for Vestibular Rehabilitation
    There are three generally accepted models that explain why this type of therapy works:
    1. Adaptation: The central vestibular mechanisms learn to adapt to the imbalanced signal coming from the impaired peripheral sensory receptors.
    2. Substitution: This is a compensatory shift that occurs when one or more sensory systems is lost or impaired.
    3. Repositioning/Liberatory/Desensitation: These are three different approaches (commonly referred to as BPPV) used to manage otolith dysfunction.

    Program Referral
    Patients can be referred to the program if they have had a pattern of clinical findings, as well as a collection of symptoms and complaints in:
    • Vestibular loss/BPPV (peripheral and central otolith)
    • Disequilibrium (central motor or movement coordination deficit
    • Central dizziness (vascular or neurological)
