Stroke Rehabilitation Program
Comprehensive Interdisciplinary approach to Stroke Rehabilitation Program and with special considerations for the Young Stroke and Senior Stroke in both Inpatient and Outpatient settings. Daily physician rounds including consultations from specialists as needed. Pharmacist and Pharmacy services are available on site. Formalized Stroke Education Series for patients and families to increase stroke awareness, prevention and increase understanding of recovery and disability from stroke. Stroke Support Group referrals are available for patients and families. Each patient receives individualized treatment plan based on the particular deficits involved, and a team of rehabilitative and medical specialists assists patients in attaining maximum function and independence.
Therapeutic Objectives
An interdisciplinary team establishes a program of care, incorporating specific methods that encourage the injured person to develop skills for problem solving approaches that are appropriate for his or her rehabilitation and continuing care.

- Assist the patient and family in emotional adjustment to the trauma of disability

Admission Criteria
Patients are evaluated upon referral from physician or case manager. The following conditions indicate patient ready for transition to acute rehabilitation program:
- The patient is medically stable, and has reasonable expectation for improvement and can tolerate intensive inpatient rehabilitation
- Has a pre-morbid condition that indicates a potential for rehabilitation
Admissions Procedure
Patients are referred for evaluation from the acute care hospital by the case manager or physician, then the referral liaison from HealthSouth does a chart review and clinical assessment of the patient, including patient interview. Patients are accepted to HealthSouth Rehabilitation Hospital of Western Massachusetts based on this evaluation in conjunction with their insurance plan provisions. Once clinically accepted and authorized by their insurance company the patient is notified of this acceptance and bed availability.
Evaluation Phase
Using an interdisciplinary approach, we identify the needs and objectives of each patient using evaluations and consultations with the physician and rehabilitation team. From this data collection, we tailor and implement a program with regular re-evaluations to further support the patient's return to functional activity.
Treatment Phase
Stroke rehabilitation is an intricate and complicated process that requires a team of experts to enhance patient's abilities on physical, cognitive and emotional levels. Our interdisciplinary team includes the following specialists:
- Medical Team - provide ongoing medical management and supervision of the rehabilitation team
- Rehabilitation Nurse - provides round-the-clock monitoring nursing care, training and education about medical needs.
- Case Manager - leads the team in providing communication between the patient, team and family, works on post-rehabilitation services and follow through with insurance case manager
- Psychological Support Team - facilitates emotional adjustment to sudden disability and transition to returning to home
- Physical Therapist - assists in increasing overall physical mobility
- Occupational Therapist - assists in achieving independence in activities of daily living
- Speech and Language Pathologist - therapy often includes communication augmentation and computer access as well as cognitive recovery and swallowing evaluation and treatment
- Respiratory Therapist - manages respiratory issues and education
- Therapeutic Recreation - uses leisure activities to augment therapy goals and provide appropriate alternative relaxation activities
