    Direct Admission Program
     
    The patients who do not require acute hospitalization may qualify for an inpatient evaluation or short inpatient stay at HealthSouth Rehabilitation Hospital of Western Massachusetts. Patients who are unable to be managed at home or who are in need of additional rehabilitation care, including family/caregiver training in safety and self-care may qualify for direct admission to our rehabilitation hospital.
     
    Common Admission Criteria
    • Severe weight-bearing joint injuries
    • Unsafe mobility
    • Proximal humerus fractures
    • Thoracic and lumbar compression fractures
    • Multiple fractures
    • Neurological conditions
    • Non-displaced pelvic fractures
    • Falling with change of functional ambulation
    • Contusions which impact ambulation status

     
    Goals
    • Improved mobility / ambulation
    • Training in self-care and activities of daily living
    • Increased strength. Endurance and range of motion
    • Assessment of home needs, including adaptive equipment or home health services
    • Family/Caregiver training
    • Instruction in home exercise program

    Emergency Room Direct Admission Program
     
    Emergency room patients who do not require acute hospitalization may qualify for an inpatient evaluation or short inpatient stay at HealthSouth Rehabilitation Hospital of Ludlow. Patients who are unable to be discharged home or who are in need of additional medical care, including family/caregiver training in safety and self-care may qualify for direct admission to our rehabilitation hospital.
     
    Common Admission Criteria

    • Proximal humerus fractures

    Referral Proccess
    HealthSouth Rehabilitation Hospital of Western Massachusetts accepts patients whose quality of life has been compromised by illness or injury. A free assessment of the patient's rehabilitation needs will be provided to determine appropriateness for inpatient, outpatient and day programs. We accept most major insurance plans.     Our Referral Team is on call on the weekends to facilitate admissions, we admit patients until 8pm.
